Pragmatic Trials: Evidence for the Real World

White Paper

While the concept of pragmatic trials has been around for decades, their adoption has accelerated over the past few years amid regulatory momentum behind real-world evidence (RWE), a maturing real-world data (RWD) infrastructure, and mounting pressure to make clinical trial populations more representative. These factors have made the pragmatic approach to trials more feasible and valuable.

FAQ

A traditional (explanatory) trial tests whether a treatment can work under ideal conditions, usually in carefully selected populations. A pragmatic trial tests whether it does work in the real-world clinical settings where it will actually be used.
The flexibility that gives pragmatic trials their real-world relevance also introduces heterogeneity, which can raise the risk of bias and confounding. The less-controlled environment can make it harder to establish clear cause-and-effect relationships.
Pragmatic trials integrate RWD such as EHRs, medical claims, lab results, registries, and CMS data to support design and execution. This includes identifying eligible patients, understanding comorbidity profiles, selecting comparators and outcomes, and simplifying follow-up.
